Acute Clinical Response Team

The Acute Clinical Response Team consists of chronic disease nurses and a GP and are employed by the Swansea NHS Trust.

The nurse looks after patients registered at surgery with serious chronic diseases.  Her care will consist of visiting patients at their home at doctors request or the request for follow-up from hospital consultants after patients have been discharged from hospital.
