You may register with the practice if you live within a two mile radius of Mayhill, Townhill, Cockett or Mount Pleasant.
To register, you will be given a registration form and a health questionnaire to complete. To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery.
You must then make an appointment with the practice nurse for a new registration check i.e height, weight, blood pressure, you will also be questioned about your medical history, any medication you are taking, any allergies you might have or any family history. All this information will make the medical team aware of your needs.
If you would like to see a doctor at the time of registration make the appointment in the time slot after your appointment with the nurse.
Students
Students currently residing at the Townhill Campus or within the catchment area of Mayhill, Townhill, Cockett or Mount Pleasant are welcome to register at the surgery.
