The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.
Before the practice can release any of your medical information you will be required to complete a Subject to Access (SAR) form which you will be able to get from reception.
Data Protection
The practice is registered with the Information Commissioner under the Data Protection Act. The information we hold will be only disclosed to other health professionals for purposes of your healthcare.
If any outside agency requires access to your notes you will be notified and we will ask you to complete a consent form from the practice.
Details of patients medical records will be accessed by all staff attached to the surgery. All practice staff have signed a confidentiality clause which is taken very seriously. No information will be given to patients relatives or representatives unless the patient has completed a carers form informing us who information should be given too.
Your Data Matters to the NHS
Your health records contain a type of data called confidential patient information. This data can be used to help with research and planning.
You can choose to stop your confidential patient information being used for research and planning. You can also make a choice for someone else like your children under the age of 13.
Your choice will only apply to the health and care system in England. This does not apply to health or care services accessed in Scotland, Wales or Northern Ireland.

- This practice handles medical records according to the laws on data protection and confidentiality.
- We share medical records with health professionals who are involved in providing you with care and treatment. This is on a need to know basis and event by event.
- Some of your data is automatically copied to the Shared Care Summary Record
- We may share some of your data with local out of hours / urgent or emergency care service
- Data about you is used to manage national screening campaigns such as Flu, Cervical cytology and Diabetes prevention.
- Data about you, usually de-identified, is used to manage the NHS and make payments.
- We share information when the law requires us to do, for instance when we are inspected or reporting certain illnesses or safeguarding vulnerable people.
- Your data is used to check the quality of care provided by the NHS.
- We may also share medical records for medical research
