As soon as you change your address, your name or change your telephone number you must call into the surgery to complete a “Change of Details” form at your earliest convenience. Failure to do this will result in us visiting, telephoning or writing to the wrong house. It is particularly important that you always provide us with an up to date telephone number, mobile phone numbers often change, if you can please provide us will your work’s number.
Remember also to notify Hospitals (if you are under investigation), dentists, central clinic if you have children of your change of address too.
